Already  well situated in the Community, Paramedics are able to provide high quality in-home monitoring to clients that have complex medical issues or a mobility compromise.  

Collaboratively working with other Community Partners we work towards ensuring clients receive the services that they require.

Goals of the CPRU

-Allow Seniors to remain at home safely-
-Decrease unnecessary 911 calls-
-Decrease Emergency Room visits and length of stay in Hospital-
-Relieve stress for family and caregivers-
-Improve the quality of life of Seniors by
keeping them actively engaged and informed-

Home Visit Program

Home Visits are conducted by Advanced Care Paramedics throughout the County of Renfrew. Paramedics complete Vital Sign, Wellness and Fall Risk Assessments, Point of Care Testing (INR), Medication Administration / Vaccinations and Blood Work.

Remote Patient Monitoring

Remote Patient Monitoring is designed for patients already diagnosed with CHF, COPD or diabetes and live at home, receive assistance when experiencing a change in health. An in-home device sends daily patient information to a system, and notifies the Paramedic when pre-determined as problematic, are collected.
